To file a program discrimination complaint, complete the USDA Program Discrimination Complaint Form, AD-3027, found online at How to File a Program Discrimination Complaint and at any USDA office or write a letter addressed to USDA and provide in the letter all of the information requested in the form. To request a copy of the complaint form, call (866) 632-9992. Submit your completed form or letter to USDA by: (1) mail: U.S. Department of Agriculture, Office of the Assistant Secretary for Civil Rights, 1400 Independence Avenue, SW, Washington, D.C. 20250-9410; (2) fax: (202) 690-7442; or (3) email: program.intake@usda.gov.

Providence Hall is committed to transparency, accountability, and ethical practices. A hotline is maintained by the Internal Audit Department of the Utah State Board of Education, and it is a venue for citizens, educators, employees, and other stakeholders to request audits or report concerns related to any of the agencies governed by or provided resources by the Utah State Board of Education.

Complaints or concerns can be filed by following the process as outlined there. Hotline complaints go directly to the State Board of Education Internal Audit Department and may be referred back to the LEA.
Providence Hall also encourages anyone concerned to reach out to the school administration when appropriate. All complaints are handled with discretion, and any individual submitting a concern may remain anonymous through the USBE hotline system.
